Why not turn it on by default and notify the user when they leave the alt text empty? And ask on the first occurrence whether they’d like to be kept notified (primary action) or they don’t care (secondary action).

This would make the functionality much more discoverable and everyone would need to make a conscious decision. The #a11y community certainly deserves this.
