'Incomplete digitisation of process': I'm reminded of an an obligatory app I had to use for a project recently. It required lots of text inputting in separate windows so no way was I using it on a phone. But photos were required and could only be attached when taken by phone, not uploaded as existing files or via desktop. Hopeless. They had not considered real-world use or the fact that they can't dictate what device a customer uses.
