@simon_brooke It was ever thus. When looking back at stuff I'd written maybe a year previously for changes/ bugfix, often thought, hmmmm that could so easily be more efficient / robust. With time pressure you just don't have the time to review everything in detail. Peer code review is a great way to go, but seldom time enough for it.
